Completed form on daily pay

Hi,

I signed up for Daily Pay the other day after getting confirmation that my old account was closed but haven't gotten any emails or anything since then...am I supposed to sign up on streamate again and then give the info to admin or does admin create an account for me? Sorry if it is a dumb question just wasn't sure
