Signing up to MFC

Hi
I'm in the process of signing up for MFC via Daily Pay, but am a bit confused (in fact am very confused!) What do I put under payment options?
I also couldn't have the same username as I have on AW as it said it was already taken, so have chosen a different one. I assume I need to email you with my new username and password for MFC?
